(WWJ) Authorities say a baby girl from Indiana slipped from her grandfather's hands and fell to her death from a Royal Caribbean cruise ship docked in Puerto Rico.

“Unfortunately the news of an accident [involving] a baby on a cruise is true,” a representative confirmed to PEOPLE.com, referring to the incident as “a real tragedy.” “The [Department of Public Safety] is working the issue with the seriousness and sensitivity required.”

Police said Monday that the grandfather of the 1-year-old girl from Indiana told officers he lost his grip while holding her outside a window on the 11th story of the Freedom of the Seas. Per the Associated Press, he was identified as Salvatorre Anello.

The incident occurred Sunday afternoon at the Panamerican dock in the capital of San Juan.

Royal Caribbean Cruises said they were helping the family.

“We are deeply saddened by yesterday’s tragic incident, and our hearts go out to the family," the cruise line said in a statement. "We’ve made our Care Team available to assist the family with any resources they need. Out of respect for their privacy, we do not plan to comment further on the incident.”

No further details were released. The investigation is ongoing.
